The New York State Cemetery Board recently approved the final version of a uniform Cremation Authorization Form which all crematories will be required to use effective March 1, 2010. Because the form is standardized, no other version of such form will be allowed to be used after that date. In addition, funeral directors will play a pivotal role in working with families to complete this form, including securing the signature to authorize the cremation.
